Diocesan Consent and Release
It’s official. We now have one form that can be downloaded and used for all parish faith formation programs. Beginning in fall 2022, this form is to be used by all parishes. If you would like it customized for your parish, please email Carmela. As of now, these forms are available, as requested last week on the parish leader check in call:
Please use this form. Do not change it. Do not copy and paste into forms you already use. Next year, this will be part of an online comprehensive registration.
